Eligibility Requirements
To qualify for Utah CHIP, a child must be under the age of 19 and reside within the state of Utah. They may not currently be covered by health insurance and must also be a US citizen or legal resident and must meet income guidelines.

Health Plan Options
The Utah CHIP program offers two plans with like benefits. One plan is administered by Molina Health Care and the other by the Public Employees Health Program.

Premium Payments, Co-Payments and Coinsurance
Eligible CHIP members may be required to pay a premium of up to $25 every 3 months. Nominal co-payments may also be required for specified services. The combination of premiums and co-payments will not exceed more than 5% of the families income.

Program Length, Enrollment and Renewal
Utah CHIP offers 12 months of continuous coverage and requires a renewal application be submitted prior to the end of the 12 month period.
