South Carolina Partners for Healthy Children (PHC)

Eligibility Requirements
To qualify for South Carolina PHC, your children must be under the age of 19 and reside within the state of South Carolina. They must be a US citizen or legal resident and provide a valid Social Security Number if they are U.S. citizens. To enroll their children, families must meet income guidelines that are based on 150% of federal poverty level.
