Eligibility Requirements
To qualify for Child Health Plus, children must be under the age of 19 and a resident of New York State. To enroll in the Child Health Plus B program, you may not have other health insurance or be eligible for the public employees' state health benefits plan. The family income must fall into the guidelines of one of the plan options. To qualify for the Plus A plan, the family income must below 160% of the federal poverty level.
Waiting Periods
Enrollment in the Child Health Plus B plan may require a six month waiting period from the termination date of prior health insurance coverage. Contact Child Health Plus for more details.

Health Plan Options
The Child Health Plus A plan is the program that is available to lower income families. The Child Health Plus B plan provides the same level of coverage, but is available to children in slightly higher income levels.
Premium Payments, Co-Payments and Coinsurance
There is no monthly premium for the Child Health Plus A plan. Families that qualify for the Plus B plan will pay a monthly premium of $9 or $15 a month per child, depending upon their income and family size. For larger families, the monthly fee is capped at three children. There are no co-payments for services under Child Health Plus.
