Eligibility Requirements
To qualify for hawk-i coverage a child must be under the age of 19 and have no other health insurance. They must also be a citizen of the United States or a qualified alien and be in a family that meets the hawk-i income limits. Children that are dependents of a state of Iowa employee or children that are eligible for Medicaid are not eligible for coverage under the hawk-i program.
Waiting Periods
There is no waiting period for the hawk-i program.

Health Plan Options
There are three health plans to choose from and one dental plan. Plan availability is determined by which county you reside in. Health insurance companies providing coverage under the hawk-i program offer plan choices that include Wellmark Classic Blue Health and Dental, Wellmark Health Plan of Iowa Blue Access and Blue Dental, John Deere Health Plan and Delta Dental.
Premium Payments, Co-Payments and Coinsurance
The premium amount that you pay is based on your family's income. Some families pay nothing and others may have to pay $10 or $20 a month for their hawk-i coverage. Native American children pay nothing. For covered medical treatments, there is no co-payment or coinsurance for most covered services. Emergency room visits may rquire a $25 co-pay.
Program Length, Enrollment and Renewal
Enrollment in the hawk-i program is for 12 months from the effective date of coverage. You may renew the health plan prior to the 12 month termination date, as long you are still meet the eligibility requirements. Children that turn age 19 become ineligible for coverage prior to their 12 month termination date.
