Colorado Child Health Plan Plus (CHP+)
Child Health Plan Plus (CHP+) is a low-cost health insurance program for uninsured Colorado children aged 18 and under and pregnant women, whose families earn too much to qualify for Medicaid, but cannot afford private insurance.
Eligibility Requirements
The CHP+ program is available to children under 18 and pregnant women aged 19 and older in need of health care in the family. Your family has to meet the income guidelines set forth and certain family expenses can be deducted from your gross income. You must be a Colorado resident, U.S. Citizen or have had an alien registration number for at least 5 years. You must not be covered under any other insurance. Applicants with household income under 200% of the Federal Poverty Level (FPL) can qualify for this coverage.
Waiting Periods
Coverage begins the same day as the application and necessary documents are received. A child must wait 3 months from the termination date of a group health insurance plan if the employer paid 50% or more of the premium.

- Immunizations
- Medical office visits
- Prescription Drugs
- Hospitalization
- Lab and Radiology
- Dental Care - Maximum benefit of $500 per year per child
- Eye glasses
- Hearing Aids
Health Plan Options
Plan availability depends on your county of residence and you must choose an HMO on the application or the state's Managed Care Network.
Premium Payments, Co-Payments and Coinsurance
Premiums are based on your household income and family size. Some families may have to pay annual enrollment fee of $25 for one child; or $35 for two or more children. You may also be required to make the following copayments:
- $2 to $5 per visit for medical care and prescriptions
- $3 to $15 per visit for emergency services
- $5 per visit for most dental services
Each year, the out-of-pocket limit that you are required to spend on health care payments for all members of your household that are enrolled in CHP+ is 5% of your annual income.
Program Length, Enrollment and Renewal
Your child's coverage last for 12 months unless they turn 19 or get other health insurance coverage during their enrollment period. You may renew coverage by completing a new eligibility application that will be mailed to you 90 days prior to your expiration date. Coverage begins on the date that the application is approved.
