Colorado Child Health Plan Plus (CHP+)

Eligibility Requirements
Health insurance provided for Colorado's uninsured children ages 18 and under whose families earn or own too much to qualify for Medicaid. The CHP+ program is also available to pregnant women in need of health care in the family. Your family has to fall within the income guidelines set forth and certain family expenses can be deducted from your gross income. You must be a U.S. Citizen or have had an alien registration number for at least 5 years.

Waiting Periods
Coverage begins the same day as the application and necessary documents are received. A child must wait 3 months from the termination date of a group health insurance plan if the employer paid 50% or more of the premium.

Plan availability depends on your county of residence and you must choose an HMO on the application or the state's Managed Care Network.

Premium Payments, Co-Payments and Coinsurance
Some families may have to pay annual fee of $25 for one child; or $35 for two or more children. You may also be required to pay between $1 and $5 for visits to the doctor or dentist.

Program Length, Enrollment and Renewal
Your child's coverage last for 8 months unless they turn 19 or gets other health insurance coverage during their enrollment period. You may renew coverage by completing a new eligibility application that will be mailed to you 90 days prior to your expiration date. Coverage begins on the date that the application is approved.
