Nationwide Lifestyle PPO Plans

There are four Lifestyle PPO plans available through Nationwide in their individual health insurance product portfolio for the state of California. Each plan provides you with 100% coverage after you have met your deductible. Some services require a continuous copayment amount that you will continue to pay, even after you have met your deductible. Plan choices include the Lifestyle 1500, Lifestyle 2500, Lifestyle 3500 and the Lifestyle 4500. Each of these plans are identical in coverage, with the exception of the deductible amount, which coincides with the plan name. For instance, the Lifestyle 1500 plan has an annual deductible of $1,500. When using non-PPO providers, you will have a 50% coinsurance, up to a $7,500 maximum. Using PPO providers is most efficient because you have access to a large physician network and receive services at a rate that has already been negotiated by Nationwide. There is also a slight difference in office visit copayments, which are $25 on the 1500 and 2500 Plans, $35 on the 3500 Plan and $45 on the 4500 plan. Office visits with non-PPO physicians are limited to a maximum payable benefit of $25.

Only the Lifestyle 4500 Plan offers maternity coverage, which is covered in full once you meet the associated $4,500 annual deductible. Emergency room visits at participating hospitals require a $100 copayment and an additional 50% of total charges is added to that amount if the hospital is not part of the CCN provider network. Prescription drugs are not subject to the plan deductible and are covered at $10 for generic, $35 for Flex Formulary and 50% for all other brands with a $50 minimum copayment requirement. All Nationwide Lifestyle PPO Plans have a $5 million lifetime maximum benefit and come with a one year rate guarantee.

You may add supplemental benefits to any Nationwide health plan, including life insurance, dental insurance, cash back supplemental benefits and discount card memberships. Nationwide health plans are portable and may be transferred to another state if you need to relocate. Nationwide contracts with the CCN Provider Network that has more than 450,000 preferred providers in all 50 states.
