Health Net offers two high-deductible PPO plans to individuals or families in their California market. These are low cost health plans that can be used in conjunction with a Health Savings Account (HSA) that allow you to make a tax deductible contribution. Funds can be withdrawn from this savings account without penalty if they are used for eligible medical expenses, including the deductible and any applicable copayments. One of the compatible plans offered by Health Net requires a minimal coinsurance after the deductible, while the other plan provides coverage in full. On any HSA health plan, the family deductible is aggregate, meaning that if you enroll in a family plan, that deductible applies to all members combined and individual deductibles are not applicable. The lifetime maximum benefit is $6,000,000 per member. Maternity benefits are not provided for either of the Health Net HSA compatible plans.
Health Net Simple Choice HSA Compatible PPO Plan
Of the two available HSA health plans, this one has the highest deductible at $4,000 for singles and $8,000 for families. All benefits, except preventive health care, are subject to this deductible. Once the deductible has been met, you are covered in full for the remainder of the calendar year and will have no coinsurance contributions. This includes full benefits out-patient prescription drug coverage. This applies to medical services received in-network. If you receive medical care outside of the Health Net provider network, you will have a 50% coinsurance after the deductible, up to a maximum of $5,000 single and $10,000. This out-of-network coinsurance maximum includes the plan's deductible. Office visits for preventive care are not subject to deductible and are available at a $40 copayment. For adults, these visits include a yearly OB/GYN exam and a prostate cancer screening exam. For hospitalization and out-patient surgical procedures with non-contracting providers, benefits are limited to $600 per day.
Health Net Smart Choice HSA Compatible PPO Plan
Offering the lower deductible at $2,500 for singles, this is the more expensive of the two HSA health plans offered by Health Net in California. The family deductible is twice that for singles at $5,000. After the deductible has been met, there is a 30% coinsurance, up to an annual out-of-pocket maximum of $4,000, which is the same as that of the Simple Choice HSA Compatible PPO plan. Preventive care is offered at a $35 copayment with the deductible waived. Hospital services under this medical insurance plan require an additional $250 admission fee, which continues to apply even after you have met your maximum. Emergency room visits have a $75 copayment which is waived if you admitted to the hospital during that visit.
