Prescription drugs are covered at $10 generic, $30 brand, with a $500 annual deductible for brand name drugs only. Maternity benefits are not covered under the 3500 Deductible PPO plan. When using a non-preferred in-network hospital an additional admission fee of $500 will apply. The out-of-pocket maximum on this plan is $3,500 in-network and $10,000 when using non-participating providers.
