Blue Cross of California HMO Health Plans

If you are looking for comprehensive health care coverage, a Blue Cross of California HMO (health maintenance organization) plan may be the right choice for you. In a Blue Cross HMO, you select a doctor to serve as your Primary Care Physician. Your Primary Care Physician coordinates all of your health care, which means that he or she treats you directly, and when necessary, manages your referral to specialists.

Blue Cross Individual HMO Plan

The Individual HMO has no annual deductible, unlimited lifetime benefits and $10 office visits. The annual out-of-pocket maximum is $3,000 per member with a two member family maximum. For most hospital and out-patient benefits, including outpatient surgery maternity, there is a 20% coinsurance up to the annual out-of-pocket maximum. This coinsurance also applies to emergency room services and inpatient hospitalization. Prescriptions are covered at $10 generic and $30 brand, with a $250 annual deductible for brand name drugs.

Blue Cross Individual HMO Saver Plan

The Individual HMO Saver plan is similar to the HMO Plan, but has $1,500 annual deductible that applies to hospital services and surgical centers. This deductible must be met before the 20% coinsurance applies toward these services, and the out-of-pocket maximum does not include the deductible. The HMO Saver has unlimited lifetime benefits and $10 Office Visits. The annual out-of-pocket maximum is $3,000 per member. Prescription drugs are $10 for generic and $30 for brand name drugs. There is a $250 annual deductible that must be met for brand drugs only.

Blue Cross Individual Select HMO Plan

The Individual Select HMO is the lowest cost HMO plan available through Blue Cross of California. This HMO plan utilizes a specific network of primary care physicians and is only available in 22 California counties. There is no annual deductible, but there is a $250 co-payment for the first four days in a hospital, per admission. The same co-payment of $250 applies per admission for out-patient surgical procedures. The Select HMO has unlimited lifetime benefits and $25 Office Visits. Prescription drugs are covered at $10 generic $30 brand.
