Capital Health Plan

History

Capital Health Plan, recognized as Tallahassee's first health maintenance organization (HMO), has a history which dates back nearly a quarter century when a group of local citizens, who recognized the areas need for better access to affordable health care, first created the plan in 1982. At that time, Capital Health Plan and the benefits provided thereunder was offered to state government employees in the Tallahassee area. Today, more than 25 years later, Capital Health Plan serves the Northern Florida areas of Leon, Gadsen, Jefferson and Wakulla counties. Offered to employer groups and Medicare beneficiaries, Capital Health Plan now serves in excess of 113,000 members from more than 3,000 employer groups and Medicare eligible beneficiaries.

Insurance Plans & Coverage

As a managed care health plan, Capital Health requires members to choose a primary care physician (PCP) from the extensive network of physicians employed by Capital Health Plan. Services are available to members at Capital Health Plan's health centers which are located at 1491 Governor's Square Boulevard and 2140 Centerville Road in Tallahassee, Florida. Benefits under the plan include preventive care, diagnostic treatments, lab tests and x-rays, hospital and surgical proceedings, inpatient and outpatient services, and emergency care treatment. Member PCPs are responsible for coordinating and managing medical care for Capital Health Plan members. In the event additional treatment is necessary, the member PCP will refer patients to participating specialists for further care and treatment. Urgent Care facilities are also available at the Centerville Road health center.

In addition to the clinical benefits offered through Capital Health Plan, members are provided with access to an array of online tools and resources, health education classes and health center discounts. To assist members in the promotion of healthier lifestyles, Capital Health Plan offers classes which address cholesterol and heart disease, CPR for infants and children, smoking cessation classes, and weight control classes. Many local area gyms provide special discounts to Capital Health Plan members as well. Recently added to Capital Health Plan's website is their state-of-the-art online resource CHP Connect. Through CHP Connect, members are able to secure online access to their personal medical records, allowing them to keep track of prescriptions, forthcoming appointments and past doctor visits. CHP Connect also allows members to search the CHP physician network to quickly obtain information about PCPs and specialists or research specific illnesses, treatments and medications. This valuable online tool allows Capital Health Plan members to better manage their health care accounts and needs.

    U.S. News & World Report and the National Committee for Quality Assurance (NCQA) lists Capital Health Plan as the top-ranked health plan in Florida. The CHP Medicare plan was ranked 4th in the nation among America's Best Health Plans of 2006.
  • May 2006
    Capital Health Plan announces innovative new program to be launched during Cover the Uninsured Week which it hopes provide access to health care benefits to employees of small business owners.
