America's Health Choice Medical Plans

History

First created in 2000, America's Health Choice Medical Plans was developed to provide Medicare Advantage plans to residents of seven Florida counties. Those counties include Broward, Indian River, St. Lucie, Okeechobee, Martin, Palm Beach, and parts of southern Brevard County. Headquartered in Vero Beach, Florida, America's Health Choice Medical Plans is a managed care health plan which offers its members Medicare Advantage Plans and a Medicare Prescription Drug Plan available to members throughout the state. One of the primary goals of America's Health Choice Medical Plans is to provide its members with access to affordable, quality health care, while offering preventive health care services, early disease detection and treatment. Membership enrollment in America's Health Choice Medical Plans currently exceeds 11,000 Medicare Advantage (HMO) members throughout its seven county service area, providing members with a provider network of 14 hospitals, 40 primary care physicians and 1085 specialists.

Insurance Plans & Coverage

Medicare Advantage plans offered through America's Health Choice Medical Plans include the Treasure Coast Premier, Treasure Coast Healthy Rewards and Treasure Coast Medicare Advantage plan designs. These plans are available to residents in Brevard, Indian River, St. Lucie, Martin, and Okeechobee counties. Available to residents of Palm Beach County are the Palm Beach Premier, Palm Beach Healthy Rewards and Palm Beach Medicare Advantage Plans. Broward county residents are provided with similar options as offered through the Broward Premier, Broward Healthy Rewards, and Broward Medicare Advantage plans. America's Health Choice Total Care plan design is available to residents in the seven service area counties.

In addition to the Medicare products offered through America's Health Choice Medical Plans, the company offers its members a variety of health and wellness classes on important health issues. Classes include instruction on weight management, diabetes, Chronic Obstructive Pulmonary Disease (COPD), Arthritis, healthy aging, fall prevention and maintaining a healthy heart. These programs encourage members to actively participate in the management of their health which contribute to improved health outcomes. Yet another program designed especially for the America's Health Choice member is the Forever Fit Program. Developed to keep members feeling fit and healthy, Forever Fit provides free fitness center memberships at participating fitness centers to members who are interested in incorporating a healthy regime of diet and exercise into their lives.
