What if I cannot remember exact information?
In rare cases, a person may have not been to a physician for several years and does not remember the name or address of that physician. In this case, you can explain this on the application. You might say, “The last time I saw a doctor was eight years ago. I do not remember the exact dates or name of the doctor.” Any additional information you can offer, such as the name of the medical clinic or the city, will be helpful. If you can remember why you went to the doctor, include this in your response as well. An insurance company is not likely to be overly concerned about an office visit that was so long ago that you do not recall the details. They are primarily concerned about your more recent visits and any diagnosis of potential health problems that resulted from that recent office visit. If you remember some details about a doctor’s office visit, but not the address and phone number of the doctor, try contacting their office for this information. You may also be able to get this information by searching an online provider directory. Most major health insurance companies have an online provider directory that is accessible to the public. Providing address and phone numbers on the application is not typically mandatory for the processing of your application. I understand that it is important to give as much information as possible but I have some problems with that, though not because of a fault on my side as far as I am concerned. I am currently in the process of applying for health insurance with Humana, as I have had them recommended from many family members and friends. I have been able to get all the dates and information in order regarding my latest visits to the doctor and I think Humana will have no problems with those as they were for minor happenings and otherwise my health is good. I do have some concerns over treatment I received while traveling abroad in Asia some years ago. I was hospitalized for some days with fever and vomiting and given some sort of antibiotic but it was never made clear to me, what exactly was wrong with me except some infection. I am worried about this and how this will be perceived as it was most likely nothing serious, but as no medical records exist, how will it look to Humana?
